Description

A facilitated incident investigation game for transport, postal and warehousing. Four cases, seventy-five minutes each, run with a room, a whiteboard and no software.

Players spend a fixed budget of investigation hours on evidence they choose. They mark up a bowtie, build a five whys chain, and sign a causal finding. The game then scores them on coverage, barrier analysis, chain quality and causal accuracy, and shows them a blame index: the proportion of their signed causes that sit with an individual.

The argument this edition makes

A consignor set a delivery window. A client changed a carton. A national standard set a throughput number. Chain of responsibility exists because the law already knows this, and what these four cases show is that the law distributes the duty and nothing distributes the information.

The four cases

• Restrained (CU-T01) — Heavy vehicle. Load restraint and chain of responsibility.
• Beam Level Three (CU-T02) — Warehousing. Racking collapse after reconfiguration.
• Peak (CU-T03) — Parcel distribution. Guard bypass against a throughput target.
• The Score (CU-T04) — Owner-driver delivery. The reporting channel as the penalty.

Facilitator pack with a full key to every case, facilitator script, quick start, student guide, printable worksheets, worked examples, a participant certificate, and the promotional card and QR code for the free browser version. Twelve files.

The game itself is free and always will be. This kit is for running it as a facilitated session with a team, which is where the learning happens. Read the facilitator key the night before, not at the lectern, or you will telegraph.

There are ten editions

This kit is the transport, postal and warehousing edition. There are ten in all and forty cases: general, mining, construction, onshore oil and gas, offshore oil and gas, transport and warehousing, agriculture, power generation, rail, and psychosocial.
