Assess with evidence, not opinion

Twenty indicators across the five conditions. Every rating records where the evidence came from. A low score built on management reports gets flagged, because that is the channel the filtering works through.

Log signals as they happen

The bypassed interlock. The deferred work order. The site visit that never left the superintendent’s side. Log it in seconds. When signals pile up against a condition you rated low, the app tells you to re-assess.

See how the conditions combine

No single condition is sufficient. Together, they are. The app flags the interactions that matter: attenuation and detachment feeding each other, production pressure ratcheting tolerance, secrecy amplifying the rest.

How to use it

From first look to a working site record in eleven steps.

Set up

  1. Explore the sample site first

    Open the app. It loads Ridgeline Processing Plant, an invented site with four of five conditions converging. Click through Overview, Assess, Actions, Signal log and Framework to see what a finished record looks like before you enter anything real.

  2. Add your own site

    Select Add your own site in the sample banner, or open Manage and choose + Add a site. Use one site per operation so the trend lines mean something. Switch between sites from the Site menu at the top. When you are ready, select Remove sample.

Assess

  1. Start an assessment

    On the Overview, select New assessment. Enter the date and the assessor’s name. Your progress saves as you go, so you can stop and pick it up later with Continue assessment.

  2. Rate the twenty indicators

    Each of the five conditions has four indicators. Rate each one Not seen, Faint, Emerging or Established. Every indicator tells you where to look. Go and look. Rate what you can evidence, not what you believe.

  3. Record where each rating came from

    Choose the evidence source for every rating. Observed in the field, Frontline worker, directly and Records or data trail count as direct evidence. Management report or dashboard and Assumption, not checked are mediated.

    A low rating built on mediated evidence gets flagged. That is exactly where attenuation hides. Test it in the field before you trust it.

  4. Put the board-level questions to leadership

    Record each answer as Substantive, evidenced, Reassuring, no evidence or Cannot answer. Judge the answer, not the tone. Three or more reassuring answers without evidence is itself a finding, and the app will say so.

  5. Save and read the Overview

    Save assessment unlocks once all twenty indicators are rated. The Overview then shows how many conditions are emerging or established at the same time, the change since your last assessment, the pattern flags where conditions are feeding each other, and a recommended next move. The count of converging conditions matters more than any single score.

Act and monitor

  1. Build the action plan

    Go to Actions. Use + New action, or pick from the intervention library and select Add to plan. Give every action an owner and a due date. Every condition rated emerging or established needs at least one.

    If an action is tagged Relies on behaviour, rewrite it. Reminders, toolbox talks and “be careful” messaging do not change the system that produced the condition.

  2. Log signals between assessments

    Select + Log a signal whenever you see something: the bypassed interlock, the deferred work order, the site visit that never left the superintendent’s side. Pick the condition, write what you saw while it is still specific, choose the source and date. When signals pile up against a condition you rated low, the Overview tells you to re-assess it now.

  3. Export the report and back up your data

    On the Overview, Export report downloads a report file you can open in any browser, print or send to leadership. Then open Manage and select Back up data. Do this after every assessment. Your data lives only in this browser, and clearing browser data erases it. Use Restore from backup to move your record to another device.

  4. Re-assess on a cycle

    The Overview shows when the next review is due and flags overdue actions. Re-assess on that date, or sooner if the signal log is running ahead of your last ratings. The dashed line on the chart is your previous assessment. Watch the direction, not the number.

It stays on your device.

Everything you enter is saved in your own browser. Nothing is sent to me or anyone else, so site findings stay inside your organisation. That also means clearing your browser data clears the app. Back up from the Manage menu after each assessment, and restore on another device when you need to.

The app opens with an invented sample site so you can see it working before you enter anything real. Remove it with one click.

Scores are a structured judgement, not a measurement. They exist so you can see a condition moving over time. The conversation they start matters more than the number.

Consistent confidence in the absence of chronic unease is a precursor to catastrophic harm.
