Description
A facilitated incident investigation game for rail. Four cases, seventy-five minutes each, run with a room, a whiteboard and no software.
Players spend a fixed budget of investigation hours on evidence they choose. They mark up a bowtie, build a five whys chain, and sign a causal finding. The game then scores them on coverage, barrier analysis, chain quality and causal accuracy, and shows them a blame index: the proportion of their signed causes that sit with an individual.
The argument this edition makes
An interlocking out for a points failure. A crossing with no active protection because it ranks sixty-first. A remote control system that takes a man out of the cab and puts him on the ballast. A rostering engine that prefers a driver for a route for an entirely correct safety reason. None of these is a story about people being careless around trains.
The four cases
- Degraded (CU-R01) — Suburban signalling. Interlocking out, two worksites, one whiteboard line.
- Passive (CU-R02) — Regional level crossing. Sighting distance and a funding order.
- The Belt Pack (CU-R03) — Rail yard shunting. Every protective function worked.
- The Same Stretch (CU-R04) — Passenger train crew. Rostered past his own incident location.
What is in the kit
Facilitator pack with a full key to every case, facilitator script, quick start, student guide, printable worksheets, worked examples, a participant certificate, and the promotional card and QR code for the free browser version. Twelve files.
The game itself is free and always will be. This kit is for running it as a facilitated session with a team, which is where the learning happens. Read the facilitator key the night before, not at the lectern, or you will telegraph.
There are ten editions
This kit is the rail edition. There are ten in all and forty cases: general, mining, construction, onshore oil and gas, offshore oil and gas, transport and warehousing, agriculture, power generation, rail, and psychosocial. Every one is $340 and every one has a free browser version you can play first. See all ten.
Running it across more than one facilitator, or putting the student material on your own LMS, needs an in-house site licence at $1,800 per edition. Licensing more than one edition is quoted on enquiry.
