Description

A facilitated incident investigation game for onshore oil and gas. Four cases, seventy-five minutes each, run with a room, a whiteboard and no software.

Players spend a fixed budget of investigation hours on evidence they choose. They mark up a bowtie, build a five whys chain, and sign a causal finding. The game then scores them on coverage, barrier analysis, chain quality and causal accuracy, and shows them a blame index: the proportion of their signed causes that sit with an individual.

The argument this edition makes

A plant can hold a best-quartile injury rate for years while the barriers that stop a release quietly degrade, because nothing in the reporting counts them. These four cases are about the gap between the numbers a board sees and the barriers a plant actually has.

The four cases

• Double Block (CU-G01) — Gas processing. Line break, isolation verification.
• To Atmosphere (CU-G02) — LPG fractionation. Relief discharge into an occupied area.
• Inert (CU-G03) — Utilities. Nitrogen asphyxiation, and not a confined space.
• The Queue (CU-G04) — Permit office. Workload on a safety-critical role.

The game itself is free and always will be. This kit is for running it as a facilitated session with a team, which is where the learning happens. Read the facilitator key the night before, not at the lectern, or you will telegraph.

There are ten editions

This kit is the onshore oil and gas edition. There are ten in all and forty cases: general, mining, construction, onshore oil and gas, offshore oil and gas, transport and warehousing, agriculture, power generation, rail, and psychosocial.
