The four cases

• CU-01 The Shutdown — mineral processing. Isolation during a night shutdown.
• CU-02 Third Storey — construction. A fall from an unprotected edge.
• CU-03 Blind Aisle — warehouse. Pedestrian and mobile plant.
• CU-04 Room 12 — aged care. Occupational violence. The hard one.

How a session runs

Seventy-five minutes per case. Teams spend a fixed budget of investigation hours on evidence they choose, commit to a hypothesis out loud at the halfway point, mark up a bowtie, build one five whys chain, and sign a finding. Then they are scored — and each team’s blame index goes on the wall next to the sentence it wrote at minute twenty.

That comparison is the session. Everything else is scaffolding.

There are ten editions

This kit is the general edition. There are ten in all and forty cases: general, mining, construction, onshore oil and gas, offshore oil and gas, transport and warehousing, agriculture, power generation, rail, and psychosocial. Every one is $340 and every one has a free browser version you can play first. See all ten.
